1. Signed printout of the online application.
2. Undergraduate (Bachelor’s) studies certificate with the Transcript of Records for the entire period of Bachelor’s studies (Legalised and translated) – original and copy
3. The certificate confirming that your Bachelor’s Diploma allows you to undertake Master’s studies in the country of issue.
Such a certificate may be issued by the equivalent of the Ministry of Education in a given country (in that case this document should be translated into Polish by a certified translator).
5. Financial Statement (only for EU candidates)
6. Medical Certificate (confirming that you’re allowed to study)
7. Polish Card – original and copy (if available)
8. Statement about citizenship
9. A copy of passport/ID Card (page with photograph)
10. 2 current photographs (35mm x 45mm)
IMPORTANT:
Failure to submit required documents implies that you lose your right to be admitted.
Any incomplete applications will be automatically rejected.
